Our new, secure electronic forms will allow you to fill out “paperwork” in the comfort of your own home at your convenience. Once you make an appointment, we will email or text you the online forms. You complete them, and submit them electronically back to us before your appointment. If you have any issues filling out the forms, please contact our office right away, as they need to be done before showing up to the office.
