Our team looks forward to seeing you. The following is a lot of information, but please read it thoroughly so there are no surprises when you come in.

Exams:

• Prior to your appointment fill out and submit the electronic intake forms we sent to you. If you have questions about the forms, please call us, and we can help you fill them out.
• Bring the following items to your exam.
  • Medical and vision insurance cards (if you did not upload them with your electronic paperwork)
  • All glasses and sunglasses
• We continue to use precautions as directed by the CDC to minimize infection.
• To minimize paper exchange, we will be sending receipts, prescriptions, rebates, and other health information through the patient portal (Revolutionphr.com). Be sure you are signed up. Please call the office if you cannot access the portal.

• Phone (434-823-4441) between 9 am - 5 pm (Monday, Tuesday, Wednesday, Thursday) and 8 am - 1 pm (Friday). Closed daily for lunch from 1:00-1:45 pm.
